Also, you will have a pay-per-visit option.
​This change will allow us to provide more personalized, attentive care and build even stronger relationships with you and your family.
We would be honored to continue caring for you under this new model.
Important: If you choose not to join, we completely understand. In that case, we kindly encourage you to reach out to your medical insurance provider for assistance in selecting a new primary care physician who best fits your family’s needs.
